
Advance Decisions
What is it? A signed document that enables you to make choices about the level of medical treatment that you would want should you suffer from a life threatening illness or condition. You can use an Advance Decision to appoint somebody to take part in medical decisions on your behalf should you be unable to do so. Within the document, you can make statements to be considered by those delivering treatment, about the treatment that you wish to receive and that which you do not wish to be given.
Who will benefit? An Advance Decision is a relatively straight forward document which becomes effective only if you are unable to participate in decisions about your medical care and at least two doctors are of the opinion that you are unlikely to recover from your condition. It will, therefore, benefit anyone, regardless of age, who wishes to have the peace of mind of knowing that their wishes are formally recorded and will be taken into account in relation to medical treatment.
